What we collect

To provide dental care safely we collect:

Identity and contact details — your name, date of birth, address, phone number and email.
Health information — your dental and relevant medical history, medications, allergies, clinical notes, treatment plans, photographs, X-rays and 3D scans.
Payment and claiming details — private health fund membership, Medicare or DVA numbers where relevant, and payment records.
Emergency and next-of-kin details, and for children, parent or guardian details.

We collect this information directly from you wherever possible — usually on your new-patient form, during your appointment, or when you contact us. Sometimes we receive it from another practitioner who refers you, or from a parent, guardian or carer.

Why we collect it

We use your information to provide dental treatment, to keep an accurate clinical record, to plan and discuss your care, to contact you about appointments and recalls, to process payments and health-fund claims, and to meet our legal and professional obligations as registered health practitioners.

We will only use your information for a purpose you would reasonably expect. We do not sell your information, and we do not use your health information for advertising.

Who we share it with

We disclose your information only where it is necessary for your care or required by law, including to:

specialists, dental laboratories and other health practitioners involved in your treatment;
your private health fund, Medicare, the Child Dental Benefits Schedule or DVA, when claiming;
our practice-management and secure IT providers, who are bound to protect it;
regulators, insurers or courts where the law requires or permits it.

Your information is stored in Australia. We do not disclose it overseas.

Our website

Our website collects far less than most. We use our own first-party visitor statistics, which are anonymous and cookieless — no name, email or IP address is stored, and we cannot identify you from them. We use Google Ads conversion measurement to count how many people click to book or tap our phone number after seeing one of our ads; this reports totals only, never who you are or anything about your health.

Online bookings are handled by our booking provider on their own secure system, under their privacy policy. Emails you send us are stored in our practice systems.

How we protect and keep your records

Records are held in secure, access-controlled systems, and only the team members involved in your care can view them. Under the Health Records Act 2001 (Vic) we must keep your health information for at least seven years after your last visit; for a patient who was a child, we keep it until they turn 25. Records are securely destroyed after that period.

Accessing or correcting your information

You can ask to see your health record or request a copy at any time, and you can ask us to correct anything inaccurate or out of date. Just contact the practice — we may ask you to put the request in writing and to confirm your identity. We will respond within a reasonable time, and there may be a fee for producing copies. In limited circumstances the law allows us to decline access; if that happens we will explain why in writing.

If you are not satisfied with our response, you can contact the Health Complaints Commissioner (Victoria) or the Office of the Australian Information Commissioner.

We review this policy from time to time. Any updated version will be published on this page.
